Application Fee for Non-OPRM Members
All Ohio Plan Risk Management (OPRM) members are automatically enrolled in the Ohio Plan Management Resources membership and do not pay additional fees.
If an entity is not an OPRM member but would like to become an Ohio Plan Management Resources member, you may choose from two available packages. (see links below for additional information)
Once the appropriate Tier Level is chosen for an entity, fill out the following application and email it to Miranda Ruark at firstname.lastname@example.org or mail to:

Once application is received, an invoice needed to complete your membership will be sent.
Ohio Plan Management Resources can provide additional services to our members at any time during membership. Costs are variable and will be determined on the specific needs of the requesting member.
